Housing Navigation & Support Services

One-to-one case management and/or educational services to prepare and assist members for stable long-term housing.  Support member for independent living skills, on-going monitoring and updating of housing support plan. Including but not limited to, assistance with providing housing resources, budgeting and crisis plans, coordinating other HOP related services, etc.  Transportation may be available for specific needs relation to this service.
